Hello guys! This is a BIG problem I've been having recently after a much needed software update which included drivers for Intel's HD 4000 IGPU.
So, after spending quite a while searching for a solution (and messing with the driver's binaries; which, btw, don't) I found this:

TL; DR:
Latest intel drivers are bugged, downgrade to 9.17.10.2932 (December 2012) and be plagued by this problem no more.

I had this error out of the blue during the beginning of the OP on Sunday. I don't have an Intel 4000 HD graphics. Using Nvidia GTX 980. Hadn't changed anything...bang..just crashed. Was able to get back on and finish OP with no issues. Windows update maybe?

When I started getting this error, it wasn't long thereafter that my GTX 960 fried.
Had to pop for a GTX 980 and I have MSI Afterburner monitoring temp and performance whenever I'm gaming. I also added two more cooling fans to the case. I have a feeling that excessive heat may have fried my older graphics card. It's loud as hell now but stays much cooler than it did before.

If you want to be sure it's not a hardware error, then what you should do is try changing the slot the card is plugged into. If you continue. To get the error then you know at least it's not your motherboard
