OBS allows the use of two push to talk hotkeys while recording, and its what I've been using for a while now with no issues. I believe you could set up something using AutoHotkey as well if you wanted, but I've never tried it.

It is indeed the SF3P-556-1 Flash Suppressor that is causing this LOD issue. Its has been fixed in the update.

While using the Legacy version I am able to swap grenades with Ctrl+G. If you swap back and are still having issues we can try and solve the problem from there.

I believe the update broke grenade swaps with the default Ctrl+G while using AGM.  You can however still use the AGM bindings which are "6" for Lethal Grenades and "7" for Non-lethal Grenades. We are also required to swap back to the Legacy version of the game temporarily and I haven't used...

Go into your AGM options ingame by hitting escape and selecting it in the top left corner. Unbind/Rebind the following two AGM bindings - Modifier Key and Show Names. I rebound mine from Left Ctrl to Right Ctrl. *Edit - CPL Avery beat me to it :P

There was an nvidia driver update around 2.5 weeks ago that was unstable with Arma 3. I know 2-3 people in my squad were having that very same issue and it went away after they reverted their drivers to the ones from 12/23/14 (347.09).

I believe the syncing issue is due to the patch that was added at the start of January. Without it you will have trouble running several maps. My advice is just download what the repository is telling you to download and you should never run into any mod issues within the unit.
