After a windows 10 update my TFAR in game isn't working and I couldn't hear anyone in TS even outside the game to just chat.  All mods are correct, not sure what changed after the Windows update or if it's even related.  Installed TS again and added plugin for TFAR as well, still no audio.  Options menu shows audio when testing my microphone out but no one can hear me and I can't hear them.  Also PTT in game does not produce the "squawk" sound when keying the mic to talk over radio.  Any advise?
 
This may be a redundant advice but have you checked whether your microphone/headphone (or speakers) are selected as your default device/default communication device in your Control Panel -> Sound -> Playback/Recording menus?
 
Just reinstalled everything.  Microphone test on TS works, no one is on currently to talk with so unknown if it is working.  Jump into the official server as I can't find patrol ops or liberation anymore...still no squawk sound when I key radio in game......
 
Stupid question, have you verified integrity game files after the update, to make sure all file are were they are supposed to be? If you have you might need to uninstall and reinstall ARMA.
 
I have run integrity again.  I hadn't tried running as admin....but I'm now speaking to you all on the other side of an OS re-install.  So I'll keep ya'll posted.  Thank you all for your input.

if you have  surround sound headphones, in teamspeak, you need to make sure the center channel, for some reason, it will be uselessly low, or inaudible.
